Offline Registration Process

1. Download the appropriate registration form from the links provided above:
   • Ward Team Registration Form
   • Departmental Team Registration Form
2. Take a printout of the downloaded form and fill in all required details manually using clear handwriting.
3. After filling the form completely, scan the filled form or click clear photos of all pages.
4. Pay the registration/entry fee by scanning the QR code provided in the registration form.
5. Take a screenshot of the successful payment transaction for verification purposes.

6. Upload both:

   • Filled Registration Form (scanned copy/photos)
   • Entry Fee Payment Screenshot

   in the upload section below and submit your registration.
